§170.314(b)(5) – Incorporate Lab Tests & Values/Results
In order to meet 170.314(b)(5), the client must have a current lab integration package with CMR to integrate with the hospital’s lab, billed as either a one-time fee or a combination of an up-front fee with an annual service charge.

170.314(b)(1) – Transitions of Care ‐Receive, Display, and Incorporate Transition of Care Referral Summaries and 170.314(b)(2) – Create and Transmit Summary Care Records
For CMR to be able to send and receive summary of care records electronically in order to meet 170.314(b)(1) and 170.314(b)(2), the client must have a current CCD integration support package with CMR, which will be billed annually.
Technical limitations: If Direct messaging is required, the client will have additional annual fees per Direct mailbox. Direct messaging is a secure, HIPAA-compliant method for transmitting protected health information (PHI). Direct mailboxes are similar to email addresses, and a different “mailbox” will be required for each “from” address. These will be provided through CMR and do not require additional contracts with outside vendors.
Other than specified above, there are no technical or contractual limitations. There are no technical or contractual limitations for CQM Solution.
